Your first visit to Southwest Foot & Ankle establishes a vital foundation for our relationship with you. During the first visit, we make sure to obtain important background information, like your medical history, and give you time to get to know your doctor. To understand what to expect for your first visit to our practice, please read through this page. You can even save some time on your first visit by printing out and completing the patient forms in advance of your appointment.

 

New Patients

Please call (435) 628-5690 or toll-free (866) 617-3668 to speak to our staff. After gathering some information, we will send you an email invitation to our secure patient portal, where you will be able to create a username and password and complete your patient registration and health history before your visit.

You may also fill out and print the Patient Registration form and bring it to your visit.

Being well-prepared for your appointment will ensure that the doctor has all of the needed information to provide the best possible care for you. It will also help relieve any unnecessary anxiety you may be feeling. Educate yourself on your symptoms by reviewing the content on this website. Also, take some time to review our staff page and familiarize yourself with the doctors. If you are able to prepare a list of medications, with dosage, as well as the last 5 years of any surgical procedures, then that will assist us in processing your information promptly. Please be prepared to provide a photo ID and insurance cards to the receptionist. We look forward to your first visit.
